The Government of the Republic of the Philippines has received a loan from the World Bank towards the cost of the Philippine Rural Development Project (PRDP) and it intends to apply part of the proceeds of this loan to pay for the cost of:

 

Supply & Delivery of One (1) Unit Utility Van for Odiongan Coco Sugar Processing

 

INSTRUCTIONS:

 

1. Bidders are required to read the instructions and fill in all the blanks properly.

 

2. Prospective Bidders/Suppliers may submit their own canvass form in accomplishing their bid proposals/quotations subject to the condition that the official bid form shall be filled up (with the offered brand, the specifications required, unit price, and total price), signed properly and attached together with the Bidder’s/Supplier’s own canvass form. (Please write legibly).

 

3. Any specification other than those required/stated in this form shall not be considered in the evaluation of bids.

 

4. Quotation/s must include all kinds of taxes for the item(s)/services listed hereunder, including delivery charges. Failure to include all relevant taxes shall be a basis for the bidder’s disqualification.

 

5. Indicate the complete specifications and brand name/model of the offered item(s) or product(s).

 

6. Price quotation/s submitted shall be valid for THIRTY (30) days from the deadline of submission of quotations.

 

7. Terms of Payment within – Thirty (30) days from the delivery of item/s.

 

8. Bids shall be submitted through any of the following:

 

1. Through sealed envelope (hand carry to the PG official address)
2. Through mail (courier) (delivered to the PG official address)
3. Through email (received at the PG official email address)

9. All entries must be typewritten or hand-written Bid Forms shall be submitted together with following documents:

 

1. Valid business permit and
2. BIR registration;

 

Note:

1. a) The bidder/supplier must be an authorized dealer, distributor or manufacturer of the goods to be procured and the brand of the product offered must be in the Philippine market for at least 10 years prior to the bid opening; and
2. b) The bidder must have after-sales service facilities in major cities in the Philippines.

 

10. Awarding shall be made on: [____] item [___] per lot [ √ ] per package. The award shall be made to the lowest evaluated quotation that complies with the minimum technical specifications and other terms and conditions stated herein.
